CPSC, Lithonia Lighting Announce Recall of Indoor HID Light Fixtures
What was recalled
These are indoor HID light fixtures with acrylic lenses and/or reflectors. They are generally used in locations such as retail spaces, light manufacturing areas, warehouses and gymnasiums. Only certain models of specific wattage lights are included in the recall. Check the Lithonia Web site for a list of the specific model and wattage combinations included. All recalled fixtures were manufactured in Crawfordsville, Ind., and have a date of manufacture from April 2002 through October 2003. The models, wattages, city and date of manufacture, and the name "Lithonia," can be found on a label attached to the ballast housing. This recall also includes certain replacement ballast kits within the above dates of manufacture.
Hazard
A component in the light fixture can leak fluid, which can degrade the acrylic lenses and reflectors, causing them to crack and fall. Falling pieces of acrylic can injure someone below the fixture.
Injuries
Lithonia has received reports of approximately 330 fixtures in which acrylic lenses and/or reflectors cracked, including reports of approximately 50 acrylic lenses, reflectors, or pieces falling from fixtures. One person suffered a laceration on his forehead, when a piece of acrylic lens fell and struck him.
